Available Services
With nearly 30 years of experience in caring for People Living with HIV or AIDS (PLWH/A) who also identify as LGBTQ+, Hope House understands the struggles and unique challenges facing the PLWH/A communities. Hope House has the knowledge and cultural competence to provide clients with uncompromised care.
We aim to assist individuals in achieving their dreams by working on their goals that focus on increasing independence. Areas of service can include household, financial, health care and time management, safety and self-care, problem solving, housing, employment, and community integration. Services offered are determined by the individual served and the support team and can be funded through private and public funding.
We serve the 7 county metro area – Washington, Hennepin, Dakota, Scott, Ramsey, Carver & Anoka. We will continue to increase as a need for services expand.
24-hour emergency assistance
This service offers a person-centered approach that includes crisis prevention support, on-call counseling and problem solving and/or immediate response for assistance at a person’s home due to health or personal emergency. Services are delivered in a wide array of options that are determined by the support team to establish the needs of the individual served.
Service options may include:
- 24-hour phone support/response
- Check-ins by phone or technology based on the need of the individual
- Weekly face to face meeting
- Technology supports
- Triage support and get 911 assistance to the person if needed
- Coordination with support team and other providers
- Person’s identified coping skills, triggers and crisis plan
- On-going assessment to ensure the person supported is receiving the appropriate level of support and needs are being met by the service.
Individualized home supports (IHS)
This service provides a person-centered approach to support and training in community living. These supports can be provided in the person’s own home or in a community setting, either in person or remotely.

- Adaptive Skills
Independent Living Skills (ILS)
This service offers a person-centered approach to support that develops, maintains and improves the community living of a person. The direct focus of this service is to provide training that increases skill development in the following areas:
- Communication skills
- Community living and mobility
- Interpersonal skills
- Reduction/elimination of challenging behavior
- Self-care
- Sensory/motor development involved in acquiring functional skills.
