Hope@Home is a comprehensive community-based service that allows individuals aging with HIV, including persons who identify as LGBTQ+, to improve and maintain their quality of life, autonomy, and self-determination as they continue to live, play and work in the home and community of their choice. With nearly 30 years of experience in caring for People Living with HIV or AIDS (PLWH/A) who also identify as LGBTQ+, Hope House understands the struggles and unique challenges facing the PLWH/A communities. Hope House has the knowledge and cultural competence to provide clients with uncompromised care.
24-hour Emergency Assistance
On-call counseling and problem solving and/or immediate response for assistance at a person’s home due to health or personal emergency.
Individualized Home Supports (IHS)
Services providing support and training in community living. Services are provided in the person’s own home or in the community. Areas of service provided include:
- Community Participation
- Health, Safety and Wellness
- Household Management
- Adaptive Skills
Independent Living Skills (ILS)
Services that provide direct training from a staff person to address the identified skill development needs of the person in areas such as:
- Community Skills
- Community Living and Mobility
- Interpersonal Skills
- Reduction/Elimination of Challenging Behavior
- Self Care
- Sensory/motor development involved in acquiring functioning
Services offered are determined by the individual served and the support team. Hope@Home services are supported through private and public funding. Contact Jill Moody with any questions about the new program.
