Strategic Plan
Board of Directors Announces New 3 Year Strategic Plan
The Board of Hope House of St. Croix Valley is proud to release a bold and visionary three-year strategic plan to continue our mission to provide a non-judgmental home with compassionate care, respecting the dignity of people living with HIV. “We knew our work supporting individuals living with HIV was still relevant”, says Thomas DeGree Board Chair. “We found through our analysis an emerging population that we could not ignore, people aging with HIV. The new Framework provides a pathway forward to develop and implement aging services for the HIV community”.
In 2019, almost 38 years since the HIV epidemic was recognized, half of those living with HIV in the U.S. are age 50 and older. Like most States, Minnesota has a growing aging adult population living with HIV. In 2017, of the over 8700 individuals living with HIV in Minnesota, 46% or 4024 individuals are over the age of 50, and 1300 are 60 or older. This is the result of effective anti-retroviral treatments (ART) which became available in the mid-90s. Due to the achievement of near normal life expectancy with ART, we are witnessing the graying of the HIV epidemic. The 2019-2022 strategic plan envisions a future where people living with HIV/AIDS can live independent as they age or have increased medical needs. Additional support services will be available to keep people in their homes and communities as long as possible. Over the next 3 years Hope House will:
- Participate in research and policy development for an emerging population of older people living with HIV by partnering with other interested organizations both locally and nationally to ensure that people with HIV have the support to live independently.
- Create programming to enable PLWH/A to age in place as independently as possible by providing services in partnership with others targeted to the specific needs of PLWH/A.
- Increase the number of PLWH/A served in the community at a range of facilities, through innovative and cost-effective service models.
