Job Opening: Community-Based Program Manager

The Community-Based- Program Manager is responsible for guiding the daily operations of the newly developed community-based aging in place program for people aging with HIV and those aging and identify as LGBTQ. The Program Manager reports to the Executive Director. This position is considered a part of the senior management team.


Responsibilities:

  • Support individuals living and aging with HIV or individuals who aging and identify as LGBTQ+ to live life to the fullest as defined by our client while maintaining optimal health, independence.
  • Manage a team of Direct Support Professionals. As the program expands, hire and manage additional staff to care for people supported through this innovative community-based Home Health program.
  • Develop and maintain positive relationships with people served, their family members, support staff, management team, consultants and other service providers.
  • In collaboration with an interdisciplinary team engage in community outreach to create and maintain a referral network regionally and throughout the State. Specifically, the Program Manager will engage in positive collaborations with leaders and colleagues within the General Aging Services, the HIV and LGBTQ+ Aging Services, and the Home and Community-Based Services Sector.
  • Process referrals, develop individualized program service plans and ensure that services are provided as designed to promote optimal health and independence utilizing person-centered planning and other theoretical approaches.
  • Administrative duties include employee management, scheduling, payroll, billing, budgeting, grant writing including maintaining documentation required to maintain the program in good standing with funders including the State of Minnesota (licensing, provider agreements, etc.)
  • Follow all pertinent organizational policies, state and federal regulations and procedures relating to the delivery of services and the general management of the program.
  • Continuously develop, improve and expand existing programming to support person-centered practices and encourage the independence of the people in your care.


Qualifications/Skills/Attributes:

  • A four-year college degree in Social Services, Nursing, or related field, master’s degree preferred and one year of full-time experience in any of the following fields: assisted living, nursing home care, adult foster care, aging in place services, medical facility, or home health care.
  • Will consider candidates with an Associates degree in social services, nursing or related field and two-year experience in any of the following fields: assisted living, nursing home care, adult foster care, aging in place services, medical facility, or home health care.
  • Excellent supervisory/leadership skills, program development, implementation, and evaluation and public speaking skills. Additionally, we are seeking an individual with experience in affordable housing, assisted living, the adult foster care system and knowledge of the local and regional social service resources which support providing aging in place services to LGBTQ+ seniors and individuals with HIV/AIDS.
  • A demonstrated desire and commitment to providing services in a cultural competent manner to individuals who are aging, who are living with HIV or have AIDS and individuals who identify as LGBTQ+.
  • Competency in using PC computer system (Microsoft Words, Access, Excel), internet and email.
  • Must have a valid driver’s license, a personal vehicle, with current auto insurance and a good to excellent driving record.
  • Must be able to pass a background check which includes fingerprinting and a drug test.
