Overview:                                      

Hope House of St. Croix Valley, with a mission to provide people experiencing life-changing health challenges access to compassionate care that respects their dignity and choices is seeking a full-time House Manager for its Adult Foster Care Program located in Stillwater, MN.

The House Manager is responsible for supervising the daily operations of the foster care home in which he/she/they work. The ideal candidate will utilize systems, such as an Microsoft Office, an Electronic Health Record, and follow all policies and procedures both internal to the organization and related to a clients funding source to ensure that the program follows applicable laws and regulations governing Home and Community-based Services. The House Manager reports to the Director of Programs and Services.

Duties Include:

  • Develop a positive relationship with people served, their family members, support staff, management team, consultant, and other service providers.
  • Support individuals living with HIV to live life to the fullest while maintaining one’s optimal health, independence, and self-determination.
  • Follow all pertinent policies, regulations, and procedures relating to the delivery of services and the general management of the organization and program. Ensure that the program operates within the budget assigned.
  • Administrative duties include employee management and supervision, hiring, scheduling, payroll, billing, budgeting, evaluation of program and employee performance, including maintaining documentation required to meet and exceed the licensing standards and expectations outlined in 245D and 9544 of the Minnesota Revised Code.
  • Process referrals, complete intake, and subsequent documents required by 245D develop and follow individualized program service plans to ensure that services provided are innovative and are designed to promote optimal health and independence.
  • Continuously develop and improve existing programming and HIV knowledge (CQA) to support person-centered practices and encourage the independence of the people in your care.

Qualifications/Skills/Attributes:

  • A demonstrated desire and commitment to providing services in a culturally competent manner to individuals with life-changing medical needs who come from diverse backgrounds including but not limited to people living and thriving with HIV individuals who are BIPOC and also may identify as LGBTQAI+.
  • Excellent supervisory, program management, and leadership skills.
  • An Associate degree in Social Services, Nursing, or related field and 3-5 years of experience in any of the following fields: assisted living, nursing home care, adult foster care, aging in place services, a medical facility, or home health and knowledge of the local and regional social service resources which support individuals with varied co-morbidities related to one’s HIV status, is preferred
  • Candidates without post-secondary education or who have a CNA license will also be considered with a minimum of 5+ years of experience.
  • Candidates regardless of education level must qualify as a designated coordinator or manager as outlined in MN statute 245D.081.
  • Competency in using a PC system (Microsoft Word, Access, Excel), internet, and email.
  • Must have a valid driver’s license, a personal vehicle, current auto insurance, and a good to excellent driving record.
  • CPR/First Aid Certification required; training provided to obtain certification.
