Founded in 1991 with a mission to provide people experiencing life-changing health challenges access to compassionate care that respects their dignity and choices is seeking a full-time Program Manager for the Hope@Home program, a community-based program designed to meet the growing needs of individuals choosing to live in place throughout Washington County.

The Program Manager’s primary responsibility is to provide leadership, management, regulatory compliance, and program implementation and evaluation for Hope@Home, a community-based home health program serving individuals with life-changing health challenges without regard to age, sex, gender identity, sexual orientation, race, color, religion, national origin, disability, protected veteran status, or ability to pay.

Primary Responsibilities:

  • In collaboration with an interdisciplinary team, engage in the successful implementation of Hope@Home, an innovative home health program designed to meet the needs of individuals with life-changing health challenges without regard to age, sex, gender identity, sexual orientation, race, color, religion, national origin, disability, protected veteran status, or ability to pay.
  • The Program Manager in collaboration with the senior Leadership team will engage in community outreach to create and maintain a robust referral network utilizing innovative marketing approaches that connects end users and referral sources to the services provided through Hope@Home. Additionally, the Program Manager will engage with leaders, decision-makers within multiple populations (HIV, LGBTQAI+, Home and Community-Based Services, and the General Aging Sectors) to activate and maintain an effective referral network while advancing one’s knowledge and understanding of the service needs of people aging or with life changing health challenges.
  • The Program Manager will manage, direct, and supervise a diverse team of Community-based Direct Support Professionals. Administrative duties include employee management and supervision, hiring, scheduling, payroll, billing, budgeting, grant writing, evaluation of program and employee performance, including maintaining documentation required to meet and exceed the quality and licensing standards and expectations outlined in 245D and 9544 of the Minnesota Revised Code
  • Process referrals, develop individualized program service plans and ensure that services provided are innovative and are designed to promote optimal health and independence.
  • Develop and maintain positive relationships with people served, their family members, support staff, management team, consultants, and other service providers.

 

Qualifications/Skills/Attributes:

  • A demonstrated desire and commitment to providing services in a culturally competent manner to individuals with life changing medical needs and who are aging within diverse populations including but not limited to people living and thriving with HIV and individuals who identify as LGBTQAI+.
  • Excellent supervisory/leadership skills, program development, implementation. evaluation, and public speaking skills.
  • Additionally, we are seeking an individual with experience in home health, affordable housing, assisted living, the adult foster care system, and knowledge of the local and regional social service resources which support individuals with health care needs and people aging in place.
  • A four-year college degree in Social Services, Nursing, or related field, 3-5 years of experience in any of the following fields: assisted living, nursing home care, adult foster care, aging in place services, medical facility, or home health care.
  • Will consider candidates with an associate degree in social services, nursing, or related field and 5+ years of experience in any of the following fields: assisted living, nursing home care, adult foster care, aging in place services, medical facility, or home health care.
  • Candidate must qualify as a designated coordinator or manager as outlined in MN statute 245D.081.
  • Competency in using a PC system (Microsoft Word, Access, Excel), internet, and email.
  • Must have a valid driver’s license, a personal vehicle, current auto insurance, and a good to excellent driving record.
  • CPR/First Aid Certification required; training provided to obtain certification.
